Abstract

Type 2 diabetes mellitus is a disease with the highest prevalence and occurs throughout life so it can increase the economic burden. In Indonesia, medical costs are covered by BPJS and expenses continue to increase every year. So cost analysis here is an important thing to consider to ensure that type 2 diabetes mellitus patients receive effective treatment. The aim of this study was to determine the cost-effectiveness of using metformin and glimepiride in BPJS patients with type 2 diabetes mellitus at Ajibarang Regional Hospital in 2023. This research is a non-Experimental observational study with a cross-sectional approach. Retrospective data collection from patient medical record data in January-December 2022. The sampling method was a total sampling with 79 patients meeting the inclusion criteria. The costs measured are the patient's direct medical costs. The effectiveness of therapy is based on fasting blood glucose levels. Cost-effectiveness calculations and analysis are carried out by looking at the average total direct medical costs, therapy effectiveness, Average Cost-Effectiveness Ratio (ACER) and Incremental Cost-Effectiveness Ratio (ICER). The results of the study showed that metformin therapy was more effective with a percentage of 22% compared to glimepiride with a percentage of 19%. The average total cost of metformin therapy is Rp. 211.294,- and glimepride therapy Rp. 215.113,-. Metformin is more cost-effective with an ACER value of Rp. 9.604,- lower than glimepiride Rp. 11.321,- and with an ICER value of Rp. -1.273,-.
